This form is for Novice Sisters & Guards to request a name tag.  Remember, the first name tag is a gift of the house.  The second and on are $15 each (prices subject to change at any time, without notice).

Double-check your spelling and ensure it is accurate.  What you enter in this form is what will be printed/etched and cannot be corrected once done, meaning you will have to purchase another to have it remade.
